My landlord gave me a custom form. How do I get that filled out by the doctor?

Make sure all your information is filled out and the doctor's section is left blank. You may then email your form to info@supportpets.com in PDF format. Or, you can visit www.supportpets.com/forms to upload your form online.

The doctor will fill out their portion, and your document will be returned via email.

Please Note: All forms are filled out at the discretion of the doctor. 

*If you live in California, we can’t complete custom landlord forms. California’s ESA law (AB 468) regulates how ESA documentation is issued, so your ESA letter is the compliant document to submit to your housing provider. If your landlord has a form, you can fill out any tenant sections and attach your ESA letter for the provider documentation.
