Why are there two different dates on my ESA forms?

Each ESA approval document includes two separate dates. The date listed at the top of each document reflects the date that the ESA approval was issued. The date labeled “License Issue Date” is just the issue date of the doctor’s license.

For further assistance, please fill out the form below and include the original email address used to purchase your ESA Documents. One of our customer service team members will get back with you as soon as possible.
