How do I get my college form filled out by the doctor?

Unfortunately, we can no longer submit college forms to the doctor.

Information regarding diagnosis and/or treatment plans is not required according to the ESA verification rules and regulations. 

Since this is due to them not having the correct forms for ESA verifications, refunds will not be given.
