Can my landlord require that the doctor fill out specific forms?
No. Landlords cannot require a health care professional to use specific forms, provide notarized documents, make statements under penalty of perjury, provide an individual’s diagnosis, and/or other detailed information about a patient’s medical history. Landlord forms are solely to confirm the ESA’s status.
In California, landlords can’t require a specific third-party form to approve an ESA accommodation. We also don’t complete landlord-provided forms for CA residents. Your AB 468-compliant ESA letter is the documentation you should provide instead.
